Tuberculosis (TB) remains a major public health challenge, but the global rise in cured survivors has unveiled a substantial proportion of individuals with chronic sequelae, defining post-TB disease. This review addresses the pathophysiological mechanisms of tissue remodeling and injury mediated by the host immune response, and proposes a framework for multimodal clinical-functional evaluation (spirometry, plethysmography, DLCO, 6MWT, and CPET) and imaging. Current therapeutic management focusing on long-acting bronchodilators, macrolides, management of fungal complications, vaccine prophylaxis, and structured pulmonary rehabilitation programs is discussed. Finally, the impact and approach to extrapulmonary complications are detailed, emphasizing severe long-term cardiovascular and neurological sequelae.
