Videothoracoscopy gained momentum in thoracic surgery from the 2000s onwards. With the results showing equivalent or superior outcomes to open surgery, the technique became popular and with the experience gained, more complex surgeries began to be performed using purely videothoracoscopic, video-assisted or hybrid techniques. . In the last method, videothoracoscopic surgery is combined with part of the surgery performed conventionally (open surgery). The advantage is that it allows the conventional part of the procedure to be performed with more economical incisions, producing less tissue damage and theoretically a more comfortable and faster postoperative recovery. We report here the case of a patient with lung cancer in the right upper lobe with invasion of the chest wall. The lobectomy was performed entirely by videothoracoscopy and this allowed the thoracic incision to be guided and positioned exactly in the area of the chest wall to be resected. The wall was rebuilt with a Marlex screen.
